A Designation of Patient Advocate is a document in which you designate someone to speak for you (your Advocate) in the event that you are unable to communicate your wishes regarding your medical care or mental health care. It covers everything from having dental work done or getting a flu shot to having surgery or life-support. Two doctors must put in writing that you are unable to participate in your own medical care or mental health care decision making before your Advocate can speak on your behalf.
