A Durable Power of Attorney is a document which allows you to designate another person (your Agent) to act on your behalf and to sign documents that requires your signature, such as Social Security forms, a Medicaid application, your income tax return, etc. The authority given to your Agent is immediately effective and it remains effective until you revoke the document or until your death.
