This document authorizes medical personnel to release medical and/or mental health records and information to people you have chosen. Such information could include copies of all records regarding your medical/ mental health history, consultations, prescriptions, treatments, x-rays, radiology reports, special consultation reports, diagnosis, and prognosis.
